Blueprint Behavior Consulting is committed to protecting the privacy and confidentiality of your personal and health information. This document describes how your information may be used and disclosed, how it is protected, and your rights regarding your information. 

Information We Collect 

We may collect contact information, demographic details, developmental and behavioral history, session notes, and communication records. This information may be considered Protected Health Information (PHI) when related to your child’s care. 

How Your Information Is Used 

Your information is used to provide services, communicate with you, develop recommendations, maintain records, and manage administrative functions including payment. 

How Your Information May Be Disclosed 

Your information will not be shared without your written permission except when required by law, for mandated reporting, in cases of risk of harm, or for necessary business operations. Only the minimum necessary information will be disclosed. 

Electronic Communication & Telehealth 

Services may involve email, forms, and telehealth. While safeguards are in place, electronic communication carries some inherent privacy risks. 

Sessions are not recorded by Blueprint Behavior Consulting unless explicitly discussed and agreed upon  in advance. If recording is ever considered for clinical or training purposes, separate written consent will  be obtained. 

How Your Information Is Protected 

We use secure, password-protected systems, limit access to necessary information, and utilize HIPAA-compliant platforms when applicable. 

Your Rights 

You have the right to access your information, request corrections, request limits on use, request confidential communication, receive a copy of this notice, and withdraw consent where applicable. 

Retention of Records 

Records are maintained according to professional standards and applicable regulations and are securely destroyed when no longer required.

If you believe your privacy rights have been violated, you may contact Blueprint Behavior Consulting at info@blueprintbehavior.com or file a complaint with the U.S. Department of Health and Human Services.
